The next step is to schedule your TABE assessments. We recommend coming into the learning center on two separate days to do these. Each session will take approximately two hours, however, if English is your second language, you will have double time. Both assessments (Reading and Math) must be completed within 30 days of each other. We currently have the following times available:

[DATES/TIMES]

Let me know which two days/times work for you. Make sure that you are feeling healthy before coming into the learning center. If you aren’t feeling well, we would be happy to reschedule your appointment. We are located at [ADDRESS]. Thank you very much, and I look forward to hearing from you!

Client-Facing Instructions Email

Hello [CLIENT NAME],

The next step in onboarding with Vermont Adult Learning is to complete the attached forms. These forms are fillable PDFs. You should be able to download them onto your computer and fill out the forms digitally. Alternatively, you may print the forms, sign them, and email me clear pictures of the signed copies. If you need to complete multiple FERPA forms, you may download multiple versions of that form. If you don’t have a computer or are unsure of how to complete the forms, please let me know and we can fill them out together in the center during your orientation instead. Here are your instructions for completing the forms:

  1. REQUIRED*: Previous High School FERPA Form: This form allows us to contact your previous high school to request your school record. Please write your name on the first line, then check the school you most recently attended. Only check ONE box per form. If your school is not included, check the box titled “Other High School (please specify)” and write your schools’ name on the line. Then, add your signature, date of birth, today’s date, the calendar year you last attended high school (if you know it), the town/state of your school, and your maiden/former name if applicable. If you are under 18, a parent/guardian will need to include their name, signature, and the date you turn 18.

*If you did not attend high school previously, or if you attended high school out of the United States, you are NOT required to do this step. Please let me know if this exception applies to you.

  1. REQUIRED: Media Release Form: This form gives you the opportunity to grant or deny permission for VAL to use photographs, recordings, videos, interviews, quotes, etc. of you in the media. Please agree or decline media usage using the checkboxes. Then, write the date, write your name, and sign your name. If you are under 18, a parent/guardian will need to write and sign their name as well.

  2. REQUIRED: Rights and Responsibilities Contract Form: This form tells you what we expect of you as a learner and what you can expect of us as your education provider. Please sign and date the form.

  3. OPTIONAL: Community Partner/Family Member FERPA Form: This form is optional. If you would like us to have contact with a community partner or family member, you will need to fill out a second FERPA form. Please write your name on the first line, then check the community partner you would like us to be allowed to contact. Only check ONE box per form. If your community partner is not included or you would like us to have contact with a specific person such as a family member, check the box titled “Other Institution/Person (please specify full name)” and write that community partner’s/person’s name on the line. Then, add your signature, date of birth, and today’s date. If you are under 18, a parent/guardian will need to include their name, signature, and the date you turn 18.

  4. READ ONLY: Student Guidelines & Understanding: Please read this form carefully. There is nothing to sign or return.

  5. OPTIONAL: Accommodations: If you require accommodations, please include documentation (IEP, 504 Plan, or Doctor’s Letter) in your reply along with your other forms.

Please send all required signed forms back to this email. Once complete, I will send you your next step.

Thank you,

Fia
